Question: Hi.
Please help,I have headaches from past two years and was diagnosed with sinusitis and was given antibiotics which gave me temporary relief but problem appeared every now and then...
Recent CT Scan of paranasal sinuses showed the following:
1)Nasal Septum showing S-shaped curvature.
2) Bilateral chronic OMU and sphenoethmoidal pattern of sinusitis
3)Concha bullosa involving bilateral middle turbinate with mucosal thickening.
What is the treatment other than surgery...Kindly advise.

Details as discussed below:

Detailed Answer:
Hi,

Thank you for your query.

1. These CT findings are common. In fact some are normal anatomical variants seem in a large percentage of the population. If you can upload or share the CT images, it will be helpful.

2. Treatment is based upon patient symptoms and not only the CT findings. The co-relation between headaches and sinusitis is less than 5%, hence care should be taken in investigating other causes of headache through a Neurologist.

3. Use Nasowash. This is a hypertonic saline nasal wash. Add half a Budesonide respule to each 200 ml of prepared solution. This will control the chronic sinusitis and help you avoid major septum and sinus surgery.

4. Medical treatment also includes an anti-histamine, an anti-leukotriene, a nasal steroid spray and regular steam inhalation. Only if these treatments do not improve the patient after 6 to 8 weeks, is any form of surgery considered.

5. Newer minimally invasive options such as Balloon Sinuplasty and radiofrequency / coblation are also available.

I hope that I have answered your query. If you have any more questions I will be available to answer them.

Regards.
