Question: Can someone interpret this for me:
'Pelviectasis involving the right kidney may be secondary to degree of UPJ obstruction'
I had right flank pain and went to get a ct. No one will tell me what this means.

Brief Answer:
There is obstruction to flow of urine from kidney due to block in UPJ

Detailed Answer:
Hi,
Thanks for writing in to us.

I have read through your query in detail.
Please find my observations below.

1. Kidneys have a pelvis which is a funnel like structure where the urine collects in the kidney and passes in to the ureter. The UPJ is pelviureteric junction and the approximate point where the pelvis and upper ureter meet.

2. Pelviectasis is bulging of the kidney pelvis and occurs when there is obstruction to the flow of urine at the UPJ or any where along the ureter.

3. In your case there is obstruction of the junction between renal pelvis and ureter, probably from an infection or a stone, if visible on CT scan. This obstruction is causing resistance to flow of urine which is called pelviectasis.

Please upload the CT scan report for accurate discussion.

Hope your query is answered.

I want to know if it is serious. My specialist appointment is not until the end of May.

Brief Answer:
Please get kidney function test (blood) done and if normal then not serious

Detailed Answer:
Hi,
Thanks for writing back with an update.

1. Some people can have a normally occurring bulging out renal pelvis and this is called extra renal pelvis. This is a common finding and normal.

2. Few can have UPJ obstruction due to a previous or ongoing infection. Your urine test is normal and the chance of an infection is less.

3. Rarely, the ureter can get affected during c section but this will also cause a hydroureter, and it is clearly mentioned that there is no hydroureter in you. Therefore this is also unlikely.

4. Stones can cause obstruction but again no stone was found in your case on CT scan.

5. It is important to do detailed tests including kidney function nuclear scan in some patients, to know if there is really any UPJ obstruction.

In conclusion, if your kidney function test, blood and urine is normal then it is not a serious condition and you can wait for May 2016 appointment. If the kidney function test is abnormal then an urgent appointment is recommended.

Hope your query is answered.

Brief Answer:
GFR is a good indicator of renal function

Detailed Answer:
Hi,
Thanks for writing back with an update.

1. GFR is one of the tests which accurately shows renal function.

2. Another test is split renal function test.

3. If your GFR is normal then there is no serious problem.

Hope your query is answered.
