Posted on Fri, 22 Jan 2016
Question: Hello
My MRI report included the following- Heterogeneous marrow signal in the left half of L4 Vertebral body with hypo intensity on T1 and T2 weighted images which may represent focal sclerosis, however neoplasticism lesion can't be completely ruled out

Hoffman testing was positive; also noted reduced sensation on my right hand side
I am suffering extreme exhaustion
Weak muscles
Unrelenting pain in my limbs which is like body toothache

What could this mean

Brief Answer:
Need to consult a neurologist.

Detailed Answer:
Hello,
Thanks for posting your query.
The focal sclerosis in the vertebra can be related to a neoplastic change. This needs to be monitored after regular intervals.
The weak muscles and generalized body ache may not be related to the lesion. This can be due to Vitamin D3 deficiency, fibromyalgia, lack of proper diet and exercise, electrolyte imbalance or a nerve related cause.
It is best to consult a neurologist and get proper investigations done to find out the cause.
I hope this answers your query.

I have been on vitamin D 3 since early November but this has not made a difference
I am alarmed that the neurologist I am being treated by simply wants to discharge me.

I know for sure that he should be monitoring me

I know that many hospitals are saying that a heterogeneous marrow signal should not be ignored as research has shown that a % of people with this diagnosis have developed malignancy

I am not panicking, but I just want proper care

Brief Answer:
Regular follow up scan is needed.

Detailed Answer:
Hello.
Thanks for writing again.
Even if your neurologist wants to discharge you, you can go for a regular follow up to monitor the lesion.
Do concentrate on having a healthy diet rich in fruits and vegetables. Regular exercise and sunbath will also help.
The aches and weakness will gradually improve.
Hope my answer is helpful.
