Brief Answer:
Of the pain and swelling is persistent then surgical repair needed.

Detailed Answer:
Hello,
Thanks for posting your queryand welcome to HCM, i went through your MRI and after thorough observation, I am of the opinion that
You have an almost complete chronic ACL tear with mild inflammation indicated by edema and mild effusion. Usually such tears heal in 6-8 weeks with rest and knee brace.
If there is a continuous pain in the knee since more than 2 months then it is best to get an arthroscopic ligament repair done for an early recovery. After a long time the chances of healing by conservative treatment decreases. Till then avoid any strenuous knee exercises.
I hope this answers your query.

Brief Answer:
Surgical repair is recommended.

Detailed Answer:
Hello.
Thanks for writing again.
I do deal with knee injuries daily in my OPD.
Try to avoid too much walking and do not carry any loads as it will add to the stress on the knee joint. The knee brace that you are using is correct but I feel that you are not giving adequate rest to the knee.
Right now a surgical repair is recommended as improper rest to the knee will not allow proper healing of the ligament.
Hope my answer is helpful.

Brief Answer:
Operation is needed.

Detailed Answer:
Surgical repair means operation. If you delay the operation or continue with your daily routine then a chronic pain can persist and it may not heal properly. After the repair, you may have to rest for 4 weeks followed by physiotherapy to restore complete range of movement.
You can consult an orthopedician in XXXXXXX at Apollo Hospital. They can do an arthroscopic surgery and fix the torn ligament. Any medicines will not help in healing.
Regards.
