Question: Hi Doctor, I had gone through the questions & answers posted in this forum & it found very informative & useful I would also like to share my information & want your feedback regarding my report as well as my wife's report. Also, I don't smoke & not drinks as well My Report as follows Semen analysis - Volume 3 ml Ph 8.0 Odour N Microscopic examination Sperm count/ml --> 15 Million/ml Sperm count per ejaculate --> 45 Million Motility(The Macleod Scale) 0 - Immobile Sperm 20% 1 - Motile with no Progressive forward movement 25% 2 - Slow forward progressive movement 40% 3 - Moderate forward progressive movement 15% 4 - Rapid forward progressive movement 00% Special Tests Fructose is Present Other Cell types in Million per ML Premature Sperm Cells -- PUS Cells Epithelial Cells -- RBC Morphology of Sperms Normal Forms - 15% Abnormal Forms 85% ------------------------------------------------------------------ Blood test Report ENDOCRINOLOGY *LH-FSH-PROLACTINE,SERUM LUTEINIZING HORMONE --> 2.25 mIU/ml FOLLICLE STIMULATING HORMONE -- > 1.74 mIU/ml PROLACTIN --> 13.96 ng/ml *Total TSH SERUM TSH --> 0.93 uIU/ml Specialised Chemistry - HORMONE TESTOSTERONE Total --> 255.52 ng/dl My age is 28 years & height is 176 cm & weight is 82 KG And we are trying for baby since 7-8 months My doctor has suggested me Maxoza sachets & Heyforte tablet to increase sperm motility & later I have suggested Fertyl M 25mg once daily for 4 weeks with Maxoza. Questions regarding my report - 1) By seeing my Semen analysis report my total motility looks 55%? please correct if I'm wrong. 2) Is my motility sufficient enough to get my wife pregnant? 3) what should be the ideal motility % in male? 4) My Rapid movement is 0% is this affecting pregnancy? 5) Can you please give your feedback on my semen analysis report? 6) What are the natural ways I can improve my sperm motilty ex any fruits, vegetables? or any medication? 7) Is the tablet I mentioned above is correct dose for me? 8) Regarding my Blood report are that normal? -------------------------------------------------------------- Wife's Report My wife age is 26 years & height is 168 cm & weight is 80 KG HSG Report a) Hysterosalpingography reveals normal size & shape of the uterus. b) There is no filling defect in the uterus nor any abnormal external impression is noted over it. c) Both the fallopian tubes are well visualised. d) The mucosal pattern of left tube is normal. e) A terminal hydrosalpinx is seen on the right side. f) Free spill of contrast in the peritoneal cavity is seen on both sides. Questions regarding my report - 1) as per above report Free Spill is seen on both sides but they have mentioned a terminal hydrosalpinx is seen on the right side So is my wife's both tubes are fine because of Free Spill? 2) Can you please give your feedback on my wife's HSG report? We both are really worried about pregnancy & will require your suggestions & advise on our report Thanks in anticipation.

Brief Answer: my comments Detailed Answer: Hello XXXXXXX I am Dr Mittal I will try to answer your query. They are quite specific.I will try to respond as specifically as possible I will start with your wife's reports. They are fine. You dont have to worry about it. that is not going to hamper her chances of pregnancy. Coming to your reports. I am afraid they are not quite well. 1) By seeing my Semen analysis report my total motility looks 55%? please correct if I'm wrong. - Ans: You see the motility percent of your report is ok, but the finer descriptions are not normal. The normal sperm percentage in your semen is quite low. if it is low, 55% percent is not going to help because the overall quantity of good sperm is low 2) Is my motility sufficient enough to get my wife pregnant? -- Ans This question is dicey. You see only one sperm is needed to get her pregnant. Now if that normal sperm reaches her ova, she will be pregnant. You do have normal sperms. only the are overcome by the slower abnormal sperms. 3) what should be the ideal motility % in male? -- Ans The motility of the sperms should be as high as possible with as low percentage of the immobile sperms and the sow moving sperms. The moderately and rapidly moving sperms are preferred. 4) My Rapid movement is 0% is this affecting pregnancy? -- Ans The rapidly moving sperms are important. They are the best possible sperms that may impregnate the ova and get you a pregnancy. However, it is not the only cause of getting ova impregnated. Slower sperms are known to impregnate too. But the chances reduce with slower sperms 5) Can you please give your feedback on my semen analysis report? -- Ans: I have already mentioned the report is not too good. I would however say that even with this report, the chance is there though slim and you need to keep trying. help should definitely be sought by experts in the field 6) What are the natural ways I can improve my sperm motilty ex any fruits, vegetables? or any medication? -- Ans: The most important thing you can do is avoid tight clothes. You see there is a reason why the testes is outside the rest of the body. It is so that they sperms can be kept at a lower temperature. Tight clothes do not permit this. So try loose clothes that give the scrotum a chance to keep the testes cooler. the indian dhoti is a good option. A good diet with proteins and minerals may help- result is inconclusive 7) Is the tablet I mentioned above is correct dose for me? -- Ans: yes the prescription is acceptable. 8) Regarding my Blood report are that normal? Yes, your blood reports look ok, and acceptable Try this information The World Health Organisation defines 'normal' sperm count as: 1. the concentration of sperm- at least 20 million/ml 2. the total volume of semen-at least 2ml 3. the total number of sperm-at least 40 million 4. Normal alive sperms- at least 75% 5. Normal form of sperms- at least 30% 6. sperms with rapid movement- at least 25%, at least 50%, if sluggish. Thanks Dr Mittal for your detail answer. I Just few points pending to discuss 1) Because of a terminal hydrosalpinx on right hand side our doctor is saying that there might be possible chance for TB on right side & doctor suggesting the Laparoscopy to check whether it is affecting the both the tubes? So should we also do Laparoscopy or not required? 2) Doctor is also saying you can go for IUI but we have started trying since 7-8 months only so we are focusing the pregnancy by natural way what is your thoughts on this? Can my sperm motility gets increase? 3) what should be the moderately and rapidly moving sperms % is required? & can I improve these % in 3-4 months? I have already stooped wearing tight cloths. 4) How can I improve my normal form of sperm %? 5) My doctor also suggested me HCG 5000 IU injection for me twice weekly for 4 weeks should it improves my motility count? 6) are there any chances to improve my sperm motility? 7) How many days sperm can alive after intercourse? 8) After ovulation how many days eggs are alive? 9) Any fruit like Orange etc can be helpful to increase sperm motility?

Brief Answer: some more information Detailed Answer: hello again XXXXXXX thanks for your confidence. 1) Because of a terminal hydrosalpinx on right hand side our doctor is saying that there might be possible chance for TB on right side & doctor suggesting the Laparoscopy to check whether it is affecting the both the tubes? So should we also do Laparoscopy or not required? ---- Ans: i think you should go for it, if you find a reliable gynecologist. there is no harm in confirming it, espeacially cause TB is treatable, and can cause problems later. but i doubt the test will be positive. but yes, its a good , safe technique. 2) Doctor is also saying you can go for IUI but we have started trying since 7-8 months only so we are focusing the pregnancy by natural way what is your thoughts on this? Can my sperm motility gets increase? --- Ans: as a doctor i always tell my patients to hold on and not to ask for medicines too much. yes, i suppose you can wait and try the normal measures and hcg. 3) what should be the moderately and rapidly moving sperms % is required? & can I improve these % in 3-4 months? I have already stooped wearing tight cloths. --- Ans: i have already uploaded the WHO standards. I think that should be the target. it might take longer than 3-4 months. we cannot predict this. we can retest in 3 months and see. 4) How can I improve my normal form of sperm %? --- Ans: try hcg. eat well. keep the scrotum loose and cool. and relax. 5) My doctor also suggested me HCG 5000 IU injection for me twice weekly for 4 weeks should it improves my motility count? ---- Ans: i would agree with your doctor on starting hcg. However if you are not in a hurry, i would have suggested a patient of mine to try natural ways like loose clothes and try again in 3 months- the repeat test- and if needed, then start hcg- i have already informed you, i prefer to avoid prescribing. 6) are there any chances to improve my sperm motility? --- Ans: as i said before, we cannot predict this. we can retest in 3 months and see. 7) How many days sperm can alive after intercourse? -- Ans: approximately a mean of 2 days. 8) After ovulation how many days eggs are alive? --- Ans: approximately a mean of 4 days. 9) Any fruit like Orange etc can be helpful to increase sperm motility? ---- try Asparagus , Avocado , Bananas , Beef , Dark Chocolate, Garlic , Ginseng , Pomegranate and Walnuts Some other tips- Avoid processed foods. Eat vegetables and whole grain.step up vitamin a and c, folic acid and proteins, may need supplements. avoid hot baths, bikes, reduce stress and weight, sleep well, avoid lubricants. Hi Doctor Thanks a lot for your detailed & positive answer. just last set of questons please. 1) I heard ayurvedic medicines are very helpful for increasing sperm motility compare to other? Whats your thought on this? 2) Do you know any good ayurvedic medicines? 3) Because of my busy schedule morning 8 am to evaining 7 pm(Mon-Fir) so I didn't get a chance to do exercise so is this also affecting the sperm motility? 4) Is there any barrier in pregnacy if wife is overweight?(My wife height 168 cm & weight is 80 Kg) 5) Is Yoga will help for me?

Brief Answer: further answers Detailed Answer: Hi again First, I have known reports where overweight can cause difficulty in pregnancy. But with a low sperm count already present, I am not sure how much it can be attributed to overweight. Besides her ideal weight is approx 70 kg. So she is not that overweight. Yes exercise is always going to help you and this includes yoga. I agree with a busy schedule it gets difficult. But I guess there is always time for a 10 min stroll after dinner, espeacially with your wife- as you said she is a bit overweight, so a little walk will help her too. Look at it this way, better a small 10 minute stroll then nothing at all. I am sorry, but as I am not trained in the system of ayurveda, I am unable to answer your first two questions this time.
