The over distended bladder and bilateral pelvicalyceal system dilatation is possible in cases of posterior urethral valves and all can be done at the moment if just follow up observation scans as interventions may lead to foetal death and abortion. Ideally the dilatation should bless than 10mm and can be monitored with serial ultrasounds. iF IT CROSSES more than that then foetus is in jeopardy and should be delivered preterm. Hence the aim should be to cross 34 weeks gestation ideally as post that chances of survival of the foetus upon preterm delivery are higher as lungs tend to be developed by then and surfactant formation is complete.
However please attach the tiff scan report for a better purview and opinion.

hello,
it has just mentioned that pelvicalyceal systems are prominent and the growth is corresponding to the period of gestation so there is no real need to be worried as the dilatation has not affected the kidneys and the growth of the child at all, so all it is advised is to follow up with serial scans. In 70 per cent of cases the pelvi-calyceal system may remain dilated but never reach a critical level and the delivery shall be normal at term and then the problem can be addressed by a paediatric surgeon after birth, so at present just monitor with ultrasound. That remains the approach.
