Question: Hello, I had a bad cold on 1st Dec, 2015. That night the cough has worsen a lot & chest hurts while coughing. Also coughed up Yellow, green & a little blood tinged sputum. Tried Azithromycin & the throat irritation gone. But I developed a dull chest pain anywhere in the chest or back. Then went to GP. He asked me to stop smoking. I am a smoker, average 12 cigarettes per day. I went to the GP ON 20th Dec, 2015. He prescribed me Clavam 625, Monti fx, Mucinac & Macberry Syrup. He asked for a Chest X-RAY (PA View). The report says -

1. No focal Lung Lesion Seen.
2. Accentuated Bronchovascular marking seen.
3. Hila are prominent.
4. costophrenic angles are clear.
5. Bony structure is normal.

Then my GP asked for another test named Sputum AFB. It also came normal "No AFB Found" for 3 days.

Now, I have reduced smoking from 12 per day to 3 per day. Also taking NRT.

Still I am having chest discomfort, dull pain while deep breathing & and also having body aches. Also having a morning cough. For temporary relief, I am using Steam inhalation.

Will you please let me know, should I get worried as it is more than 1 month now. But still I have the chest discomfort. Does the tests and symptoms are indicating any serious or malignant disease?

Eagerly waiting for your reply.

Brief Answer:
No need to worry for any serious or malignant disease.

Detailed Answer:
Thanks for your question on Health Care Magic.
I can understand your concern.
You are having chest symptoms and hemoptysis (blood in sputum).
These are common in bronchitis, lung infection and lung cancer.
Since your chest x ray is not showing any lesion, no need to worry for lung infection and lung cancer.
Accentuated bronchovascular markings, prominent hila on chest x ray with hemoptysis in a smoker patient is more suggestive of bronchitis.
So better to consult pulmonologist and get done clinical examination of respiratory system and PFT (Pulmonary Function Test).
PFT will also tell you about severity of the disease and treatment is based on severity only.
You may need inhaled bronchodilators and inhaled corticosteroid (ICS).
Smoking cessation should be done first.
Don't worry, you will be alright. Nothing to worry about serious or malignant disease.
Consult pulmonologist and discuss all these.
Hope I have solved your query. I will be happy to help you further. Wish you good health. Thanks.

Thanks, Dr. Bhavsar.

This problem starts when the weather is cold & dusty, i.e. in the winter. I also have a dust allergy. Will you please let me know, why I am having a chest discomfort like tightness & dull pain in chest & sometimes in the back? Sometime I also have a pressure feeling in my left temporal area with mild headache. How much time will it take to recover completely?

Brief Answer:
Recovery depends on severity of your disease.

Detailed Answer:
Thanks for your follow up question on Health Care Magic.
I can understand your concern.
Eecovery depends on diagnosis and it's severity.
You need to first confirm that you are having bronchitis. And PFT will tell you about severity of the disease.
With smoking cessation and inhaled treatment, usual time of recovery is 1 month.
So better to consult pulmonologist as soon as possible and start appropriate treatment.
Please let me know
1. Do you have breathlessness on exertion?
Your all symptoms are due to airway inflammation because of cigarette smoke. This causes bronchospasm. Bronchospasm causes chest tightness, cough, pain etc.
So please let me know if you have breathlessness.
I will be happy to help you further. Wish you good health. Thanks.

Hello Dr. Bhavsar

Thanks for your response. Yes, sometimes I am having breathlessness after having some physical activity. The chest pain is the most irritating now. Also the dusty environment of winter is affecting this. Will you please let me know, bronchospasm is dangerous or life threatening? What can be done? I am really worried. Sometimes I also feel constant spasm in chest (both side). But its rare.

I am really worried & also confused for the same. Will you please let me know what can be done?

Brief Answer:
Consult pulmonologist and get done PFT.

Detailed Answer:
Thanks for your follow up question on Health Care Magic.
I can understand your concern.

Bronchospasm is not life threatening but it should be treated promptly whenever present. The reason is that it can cause obstruction to airflow leading to carbon dioxide retention. This can lead to some complications.

By your history and description, possibility of bronchitis is more.
But we need to confirm this.
And for this PFT (Pulmonary Function Test) is must.
PFT will not only diagnose bronchitis but it will also tell you about severity of the disease.
And treatment of bronchitis is based on severity only.

According to the severity, you may need inhaled bronchodilators and inhaled corticosteroid (ICS).
Please let me know.
1. Have you ever consulted pulmonologist?
2. Have you ever undergone PFT?

So please reply me answers of above asked questions, so that I can guide you better. I will be happy to help you further. Wish you good health. Thanks.
