What Does My Abdominal Ultrasound Scan Report Indicate?

Question: Dear Dr. I am 40 years old, not married no children, i did a general medical check up which included tumor markers. The results came up with elevated CA results. i was asked to do Abdomen scans which i did. My results came out with Uterus Bulky measuring (13.83 x 9.82 x 9.78 cm, contains multiple well defined hypoechoic lesions largest measuring 4.32x4.19 cm. Some lesions are seen abutting on the endometrial lining. My endometrium is normal, Rt ovary measures (3.71 x 2.62 x 3.49 cm contains a cyst measuring 2.98 x 2.17cm. What do these results mean, how bad are they, what do i need to do?

I am Dr Ramadevi Wani. I will be answering your concerns today.

I read your ultrasound report. From the report you seem to be having fibroids in the uterus. Uterus is made up of muscle cells. Fibroids are simple growths of muscle cells. They are not cancer. The possibility of fibroids becoming cancer is extremely rare. The fibroids can cause heavy periods. Do you have heavy periods?

When was your last period? If this ultrasound was done just before your period, the cyst in the ovary could be a corpus luteal cyst (cyst formed after ovulation). It is important to repeat the ultrasound in the first week of your period cycle to see if it has disappeared or not.
You did not write about the result of CA 125. It is important to know how high it is?
CA 125 can be high in some cancers ( cancer ovary, colon, lungs etc). It can also be high in certain non cancerous conditions like fibroid uterus, endometriosis, mensus, infection etc.

Do write to me about the level of CA 125 and the normal range for your lab so that I can help you further

My periods are very normal not heavy , second day abit heavier than first but not out of the ordinary. They are also regular, always happen when i expect them,my cycle is an average 27 days, the last ended on 31.01.16. My periods last two days. The scan was done on 15.01.16.

Scan was done after my period.

The lab tests were done during my period a day into my period that is on 30.01.16.

I must have ovulated on 12.01.16 scan was done on 15.01.16

My CA 15-3 was 41.4 and my CA 125 was 82.4 for markers up to 35.

Welcome back.
You have done the CA 125 during your periods. In addition you also have fibroids.
These two factors may have caused increased CA 125 levels. This CA 125 is not very high. The level is usually in hundreds in the presence of cancer.

I suggest you repeat the ultrasound for ovaries in the first week of period cycle after bleeding stops. Repeat the CA 125 too at that time. If the ultrasound is normal and CA 125 level is decreasing there is no reason for concern.

I do not think that ovarian cyst is the cause for raised CA-125 in your case. It is probably raised because of fibroid uterus and mensus.

Bulky uterus means enlarged uterus. It is enlarged because of fibroids.
