is this all caused by diabetes

Diabetes cannot cause anemia directly nor any of the other changes on the CBC.
The elevated alkaline phosphatase can have several reasons. Two examples are a recent fracture, and vitamin D deficiency.
When I see someone like you in my practice, I typically order the following blood tests in addition to a detailed physical examination:
HbA1c (Glycosylated Hemoglobin = your 3 month glucose average)
TSH
12 hour Fasting Lipid profile
Urine albumin to Creatinine Ratio
25 hydroxy Vitamin D

Regarding the alkaline phosphatase level, I have two complete tears, two partial tears of my rotator cuff along with a bicep tendon tear. Could this possibly be the cause?
This was a pre-op appointment for getting my rotator cuff repaired. After seeing the cbc, I was a bit concerned about getting these levels fixed before the surgery.
What really surprised me is that the doctor did not mention them; but, of course, the PA did the exam and everything else. My ekg was great and bp was only 113/63 ....I only take the micardis 1x every other morning.
