What Does My Cutaneous T-cell Lymphoma Workup Suggest?

Question: Cutaneous t cell lymphoma panel shows 53% cd4 cd30 dual positive cells, along with an absolute count of over 2300. Also noted was a 3% cd4 cd25 dual positive and absolute count of 153. I was previosulr diagnosed with Mycosis Fungoides last December. Is this a sign of Anaplastic Large Cell Lymphoma? CBC looks good except for mild lymphocytosis, and some reaxtive lymphocytes found of an absolute count of 1000, and 9.6 %. Also all cd 4, cd8, cd 5, and cd3 cells counts are elevated, and gave been since last year.

Is there any other reasons CD4 / cd30 dual positive cells would show up in a flow cytometry ? Is there a normal reference range? Or should there be none?

Normal reference range for the disease to be postive varies from 25% to 66% and in your case it is 53%.

What confirms the diagnosis ? Gene rearrangement ? I dont have any obvious swollen lymph nodes...

For confirmation of diagnosis you need to do fluorescence in situ hybridization (FISH) test on your blood or bone marrow cells to see if there is anaplastic lymphoma kinase (ALK) gene rearrangement and ALK protein expression.
