What Does My Cutaneous T-cell Lymphoma Workup Suggest?
Question: Cutaneous t cell lymphoma panel shows 53% cd4 cd30 dual positive cells, along with an absolute count of over 2300. Also noted was a 3% cd4 cd25 dual positive and absolute count of 153. I was previosulr diagnosed with Mycosis Fungoides last December. Is this a sign of Anaplastic Large Cell Lymphoma? CBC looks good except for mild lymphocytosis, and some reaxtive lymphocytes found of an absolute count of 1000, and 9.6 %. Also all cd 4, cd8, cd 5, and cd3 cells counts are elevated, and gave been since last year.
Brief Answer:
CD30 antigen positive
Detailed Answer:
Hi
Since your cutaneous t cell lymphoma panel shows 53% cd4 cd30 dual positive cells and you are having absolute count of over 2300 it could be a sign of Anaplastic Large Cell Lymphoma.
General symptoms of this disease may include loss of appetite, night sweats, weight loss and tiredness.
The presence of the protein CD30 antigen on the surface of lymphoma cells is the hallmark of the disease.

Is there any other reasons CD4 / cd30 dual positive cells would show up in a flow cytometry ? Is there a normal reference range? Or should there be none?
Brief Answer:
25% to 66%
Detailed Answer:
Hi
CD4/cd 30 dual positive cells in flow cytometry is only caused by anaplastic large cell lymphoma.
Normal reference range for the disease to be postive varies from 25% to 66% and in your case it is 53%.

What confirms the diagnosis ? Gene rearrangement ? I dont have any obvious swollen lymph nodes...
Brief Answer:
anaplastic lymphoma kinase positivity
Detailed Answer:
Hi
For confirmation of diagnosis you need to do fluorescence in situ hybridization (FISH) test on your blood or bone marrow cells to see if there is anaplastic lymphoma kinase (ALK) gene rearrangement and ALK protein expression.

What would you put the percentage of having it based on the flow cytometry data alone?
Brief Answer:
60%
Detailed Answer:
Hi
Based on the flow cytometry data percentage of having anaplastic large cell lymphoma would be around 60%.
