Brief Answer:
Can be precancerous lesion, go for biopsy

Detailed Answer:
Hi

Thanks for writing in.

As per your history & reports, it can be premalignant lesion or condition like:-
- leukoplakia
- candidiasis
- oral submucous fibrosis
and many more...
Since you have history of smoking, i would suggest you to strictly quit smoking first.
Take multivitamin with lycopene once daily for 1 month minimum & then observe if there is any change in lesion.
For proper diagnosis go for biopsy.
Consult your dentist for further opinion.

Hope I answered your query

Wishing you good health

Brief Answer:
Can be precancerous, get biopsy done

Detailed Answer:
Hi

It looks like precancerous lesion.
Smoking is not the only reason, there can be various other reasons like improper hygiene, tobacco or betel nut chewing etc.
Moreover any lesion present for more than 1 month is indicated for biopsy.
Need not worry & go for biopsy to get accurate diagnosis.
Chances of cancer are rare in my opinion.
