What Does My Liver Function Profile Indicate?

Question: I got my complete health checkup done and it shows little abnormality in Liver Function Profile. Can you please advise what could be the cause for this? And what should I do?

I am Dr.R.K and I am pleased to assist you.

Your liver enzymes are elevated. Of these the elevation of SGPT is very significant.
I presume you are not any liver toxic medications.
I also presume you are without any symptoms.

The most common cause of raised SGPT is nonalcoholic fatty liver disease (NAFLD). And the most common cause of NAFLD is obesity.
But your ultrasound scan report shows a normal liver.
From your other post I could make out you are overweight. Reduce weight and try to maintain a BMI of less than 25.

I would advise you to repeat the liver function test in an another lab.
If the repeat test is also abnormal, more tests are to be done to find the cause.
The following should be done as the next step -
1. HBsAg
2. Anti HCV
3. Serum ceruloplasmin
4. Autoimmune profile

Yes you are correct. I am not under any medication currently. Not sure what the symptoms could be.

I went for a checkup to physician and below is her diagnosis:
1. Abnormal Liver functions for evaluation. R/o NAFLD, R/O AI disease.
2. Dyslipidemia
3. Polyarthralgias & low back pain
4. Microscopic, hypochromic anemia.

Below are her recommendations:
1. Do tests - ANA panel, Viral Screen, HbA1c & Iron Panel. Do LFT again after 2 weeks.
2. Diet: Low carb, fibre rich diet.
3. Add walnuts & flax seeds
4. Daily moderate exercise for 45-60 mts.
5. No medication for now.

Questions:
1. Could PCOD & Liver function abnormality be related?
2. What could be the symptoms? I do feel hungry. Don't feel like vomiting or nauseated.
3. CBC shows Microscopic, hypochromic anemia. What is this and what could be the cause? What should I do to take care of this?
4. What is ESR & what could cause its elevation? What should I do to take care of this?
5. Lipid profile is also on borderline. What should I do to take care of this?

I have been having a healthy diet since 3 months so not able to understand the cause of all this. My height is 155 cms & current weight is 63 kg. What should be my ideal weight?

Good to see your doctor has mentioned the same (NAFLD Autommune causes, viral screen, repeating LFT etc).

1. PCOD does not directly affect liver. But PCOD XXXXXXX be associated with NAFLD which can affect liver function.

2. Symptoms of liver disease include jaundice,abdominal distension, leg swelling etc

3. Microscopic hypochromic anemia means you have low hemoglobin probably due to iron deficiency though there are other causes as well. Check your Ferritin and iron levels. If low, take iron supplements and one dose of albendazole. Take iron rich food like greens, apple, dates etc

4. ESR is a marker of inflammation. When there is infection or inflammation anywhere in the body it raises Treat the underlying cause. Anemia also increases it.

5. Decrease fatty food and regular physical exercise are enough to control it if it borderline.

BMI Calculators are available online.

Your present BMI is 26.22.
Try to keep it under 25. (If your weight is below 58 kg, your BMI would be less than 25).

1. So what should be my next course of action?
a. Shall I wait for 2 weeks & then go for repeat-LFT or should I do it now again in another lab?
b. Or I should do these tests first - ANA panel, Viral Screen, HbA1c & Iron Panel.?

2. Also, regarding diet should I stop eating below food items or in what quantity should I take it?
a. Milk
b. Curd
c. Green Tea
d. Mushroom
e. Fish

3. I am attaching the diet plan that I have been following. Can you check it once and see if any particular food is aggravating it or if I need to stop something immediately?

4. How serious is this? Is this curable? I am scared. I do not have jaundice, abdominal distension & leg swelling also. I was planning for pregnancy also from March '17.

5. Since last 2 years I gained weight from 53-66. Then from November after I started following diet & exercise, my weight reduced to 63 now.

You can repeat the LFT after 2 weeks in a standard lab.
There is no restriction in the food items you have mentioned.

Regarding the diet , you can post a question separately and our dietician will respond.

There is no need to worry. Treatment may be planned after the repeat LFT.
