Brief Answer:
Suggestive of poor heart function ? ischemic heart disease

Detailed Answer:
Hi & Welcome.
I can understand your concern.
A "Positive TMT" indicates that your heart started getting tired (difficult to perform its normal activity) due to ischemia (reduced blood supply to heart) with stress induced by increase in exercise in the form of running.
Since you have a history of high BP you might be at a risk for heart attack.
It is a matter of concern but not too much.
I would therefore advise you to better undergo Angiography at once. Angiogram is a screening procedure to find any abnormalities in the blood vessels of the heart.

Besides, had you been my patient I would have advised certain life style modifications like:

1 Daily regular exercise (like early morning walking)
2. Avoid smoking or taking alcohol (if any)
3. Avoid fatty meals
4. Avoiding anxiety and excessive worry just enjoy your life
5. Reduce body weight if you are obese
6. See that your BP is controlled with the prescribed medications (reduce your dietary salt intake).
7. See that your lipid profile (serum Cholesterol, Triglycerides are within the normal ranges)

Hope the suggestions given above could be useful.
Do reply if you have any follow up query.
Best regards.
