What Does My Ultrasound Scan Report Indicate?

Question: I was trying to interpret my transvaginal ultrasound impression to determine what is going on with me. They include ovoid mid endometrial body nodule measuring 7 x 3 x 5 mm, differential includes polyp, focal hypertrophy, and nodule. 1.3 cm right ovarian cystic nodule with subtle wall regularity - recommend short-term follow up ultrasound or MRI. and 2.3 cm anterior uterine body intramural leiomyoma. What does this mean?

1. There is a 5 x 3 x 5 mm area in the cavity of uterus as visualized on ultrasound and this can be a polyp or growth arising from the lining of the uterus.

2. There is a 23 x 22 x 18 mm fibroid in the wall of uterus anteriorly. This is a small fibroid and not a serious problem.

3. There is a fluid containing cyst measuring 13 mm in right ovary with suspicious wall irregularity.

The endometrial polyp and uterine fibroid findings are likely benign but the cystic nodule with suspicious wall irregularity in right ovary requires close monitoring like an ultrasound in 3 months and followed by MRI scan if needed. Please discuss with your doctor and get certain blood tests for ovary done if required.

Thank you for your prompt response, and helping me understand this better. Could these results be indicative of endometriosis? And how quickly should I follow up with an OBGYN, as I am experiencing constant pelvic pain and pressure and general discomfort, urinary frequency, nausea, and extreme fatigue?

1. The endometrial polyp and uterine fibroid are not endometriosis. However the cyst in the ovary needs to be followed up in 6 to 12 weeks to know if it can be a endometrioma or chocolate cyst which is a rare possibility. An endometrioma is a focus of endometriosis in the ovary. Right now it is difficult to tell if the cyst with irregular walls in the right ovary is related to endometriosis.

2. It is important to get another urine culture and sensitivity test after completing the antibiotics. This will tell if your infection is treated or you need any new medicine. I am not aware if the antibiotics were prescribed by your OB or another doctor.

3. The scans can be repeated after 3 months or earlier. Please be informed about the consequences of taking norco pain relief. You can switch to mild pain relievers if the pain is tolerable.

The antibiotic Flagyl was prescribed by my general practitioner as I do not have an OBGYN as of now. This medication has been making me feel very ill, and I am only starting my 3rd day of treatment. I am only taking the Norco for pain as needed, when Ibuprofen does not work, and I do understand the consequences that the Norco can have on liver function.

A final question regarding my labs. My ALT came back at 5 with +15 ketones in my urine. Does this mean anything worrisome combined with my radiological findings?

1. The antibiotic flagyl contains metronidazole and this medicine is known to cause loss of taste in many patients and probably the reason why you feel sick. This will stay till you have the medicine.

2. It is good that you know the adverse effects of norco and using it properly.

3. Your ALT is low and should be evaluated with other results. In itself it is not a problem. Ketonuria occurs in those having less carbohydrates or any condition with cardohydrate metabolism. If it persists then please discuss with your doctor.
