Brief Answer:
Both diagnosis need different investigations

Detailed Answer:
Hello , welcome to HCM.

I would like to explain few facts.
When we do xray shoulder we can see part of lung .We can not visualized entire lung. Every organ has different xray views and different factors for finding pathology.

Secondly commenting on lung problem with shoulder xray is not always accurate.
In sonography of shoulder it will not be possible to find lung problem at all. Even we do lung sonography , it is difficult to find lung tumors.

In your case i would like to suggest do proper chest xray and if it shows something then do HRCT THORAX investigation. Later investigation is gold standard and 100% accurate to find lung issues.

I would also like to suggest please take opinion of radiologist after chest xray.

Hope this may help you .
Please feel free to ask any further query.
