Question: Hi Sir,
I am 28 year old male suffering from chronic hepatitis b for last 16 years..my recent USG report shows coarse echotexture of parenchyma continuously..my HBe Ag is negative and HBV DNA quantative report today reads as 323 IU/ml..kindly guide how serious my HBV DNA quantative is presently..and what is the normal range for it..whether i need bed rest..and whether i am suffering from liver chirohsis or likely to suffer till when

I understand your concerns
Well need to look at your liver function tests? Do you endoscopy report if any?
Also are you on any medications?
Unlikely you have cirrhosis but worthwhile getting an fibroscan done to look for same, this is much definite test.
Your viral load is low so that's good but there is a possibility that this low viral load is causing some problems in liver that needs to be further evaluated. Also you do not need any bed rest.

My SGOT is 128 and SGPT is 150 and serum bilrubin is 1.6( 6 days ago)...i am getting mild fever and headache continuously at night..apetite is fine...and was taking ADESERA 10mg once a day when 10 years ago my SGOT/SGPT levels were in 400 range and my HBV DNA report was 2 lacs/ml...i continued this medicine for 4 years then i was fine...can i continue this medicine once again as my SGOT/SGPT Levels are high again...my liver ultrasound report is showing coarse echotexture of parenchyma.

Well, you need to be restarted on antivirals.
You should not have stopped Adsera (Did you consult any doctor before stopping?)

You need to be started on probably more potent antiviral now . Need to undergo an endoscopy to look for changes of cirrhosis and fibroscan if available to look for same.

You need to be on regular follow up with a gastroenterologist and be compliant with your medications

Hello Sir..
I visited doc in XXXXXXX ..various tests were done...my fibroscan report of liver is 9.1 Kpa and CAP of 163..my lipid profile is abnormal...sgpt is 122..sgot is 59..and anti Hbe is positive
Sir whether i am suffering from liver cirrohsis if yest then how serious it is as my viral load report is 323 IU/ml only..my doc has presribed Ricovir 300mg once a day...Ursocol 300mg twice a day and Alkasol two table spoon at night for 6 months as i am having UTI also .
Sir should i take above medicines looking at the fact that my viral load is only 323 IU/ml and my HBeAg is negative and anti HBeAg is positive.....kindly guide me with true facts of my cirrohsis condition as i am really confused as what should i do...Also i am continuously feeling feverish especially at night from 15 days which may be due to UTI...

Hi,
Good you consulted an gastroenterologist.

He has started you on a potent antiviral which you need. Don't stop if unless your doctor asks you to do so. Though your viral load is low it is actively damaging your liver to needs to be treated.

Possibly you have significant fibrosis or early changes of cirrhosis on the basis of your fibroscan report so you need to take the medications prescribed by him.

continue your antibiotics and keep monitoring temperature using a thermometer. If your are running fever it is unlikely due to hepatitis.

Sir,
I am greatfull to you Sir for your precious guidance...i just need to ask you that my HBeAg is negative and anti HBeAg is positive..then is it safe for me to start on a potent anti viral treatment..will it not have any side effects..when i asked my doctor then he said that my report is wrong as accordingly him my HBeAg report can not be negative as i am suffering from fibrosis and my liver is damaged...he has also asked me to get tested for Alfa feto Protein test after six months..why is AFP necessary and what is it for....kindly guide Sir..

you definitely need potent antivirals as there is fibrosis in your liver, IRS side effects are minimum and benefits are much more.
e antigen can be negative there is nothing unusual about it and to me report looks OK and its good it is negative.
