What Does Protein,leucocyte And Blood In Urine Suggest?

Question: Hi my 9 month old baby has 3 plus lucacites 3plus protein 3 plus blood in her urine but test came back no growths ... She has had temperatures , she has mucas as urine she is ill looking very very white very dark eyes sleeps a lot drinks a lot , nose bleeds but no one seems to give me answers to why this is happening

Brief Answer: Most likely to be urinary tract infection Detailed Answer: Hi Welcome to Healthcare-Magic Greetings of the day Dear Ms XXXXXXX The presence of protein, leucocyte and blood in Urine suggest the possibility of Urinary Tract Infection. Many of the times the culture could be negative because of sterile pyuria or use of antibiotic even before urine culture specimen is taken. Presence of fever and ill look suggest possibility of Febrile Urinary tract infection. Remote possibility of Nephrotic syndrome is also to be considered. I would suggest you to consult Pediatrician/Pediatric Surgeon and get the following investigation 1. Complete Blood count 2. Serum Creatinine 3. Plasma protein/Urine protein ratio & C-Reactive protein(CRP) 4. Serum lipid profile 5. Serum Creatinine, Blood Urea and Blood Urea Nitrogen 6. Ultrasound of Kidney , Ureter and bladder to rule out involvement of kidney cause by ascending infection from urinary bladder 7. Blood pressure measurment It would be more helpful if you get back to me with her complete history , complaints, Investigation, and treatment been given. Do not worry , things can be taken care off. Brief Answer: Sterile UTI still possible Detailed Answer: Hi Greetings I see quite a number of babies with such investigation profile, clinical look like Urinary tract infection but cultures are negative. They require further workup by getting Ultrasound abdomen and other blood investigations as previously suggested. They turn out to be urinary tract infection and they do respond to antibiotics. Do get back to me with detailed reports as available with you. I would also suggest getting the investigation done as advised , in consultation with your Pediatrician. Take Care Regards Dr Deepak
