What Does Severe Upper Abdominal Pain After Hiatal Hernia Surgery Indicate?

Question: My son had surgery to correct a huge hiatal hernia. I am having severe upper abdominal pain, especially starting about 4:00 pm. Taking a big dose of Gaviscon seems to help eventually. These are same problems my son had as well as my father. In his day it was bad surgery experience..broken ribs, etc. have I inherited the same condition, you think?

There are various reports and studies all over the world which suggest that familial inheritance of hiatus hernia does occur.
There are various types of hiatus hernia and can be of varying severity.The symptoms depend on the type and severity of defect.
It may remain asymptomatic,can cause acidity and reflux or even bleeding or ulcer.Barium study( chest x ray done after barium swallow) and/or upper GI endoscopy helps in diagnosis.
In a symptomatic person in the presence of positive familial history screening should be done to rule out hiatus hernia.Although there are many other causes apart from hiatus hernia and Gaviscon and prisolec helps in releiving the symptoms in all these condition,I would suggest you consult a physician and discuss about the need of screening.

What can I do to prevent further episodes? I drink one cup coffee daily. I am not overweight. The pain begins about half hour after small evening meal. 5:00 pm.

You can try esomeprazole and domperidone combination

Prisolac contains omeprazole a proton pump inhibitors(PPI).Now a days more potent newer generation PPI are available like esomeprazole and lansoprazole.
Addition of prokinetic agent like domperidone,which increases the forward motility and prevent reflux helps in relieving the symptoms.Continue Gaviscon as usual.Both these are prescription medication so please get it prescribed by your physician.I would suggest you to stop the consumption of coffee as this precipitates the symptoms.
Sometimes infection of H pylori causes worsening and failure to improvement in symptoms.Antibiotic treatment is needed in the case of infection with H pylori.

I would suggest you to consult your physician and discuss about the need of upper GI endoscopy to rule out hiatus hernia and H pylori infection.
