What Does "small Right Paracentral Disc Herniation" Mean?
Question: I have been experiencing pain in my lower back for the past two years. MRI showed herniated disc at L5S1. I saw a neurologist who said that pain was caused from facet not from disc. I took anti inflammatory meds, had facet shots and have done pt. Pain is worse in the morning after sleep (on left side). Recently I have been experiencing a severe burning sensation on the right side. New MRI results showed disc desiccation at every level and a small right paracentral disc herniation which abuts S1 nerve root origin. Disc space narrowing at L5S1.Can you explain what this means and what the best course of treatment would be?

As per your new MRI there is nerve root compression which may be important reason for pain.
Only disc space reduction at l5 s1may not be important reason for pain.
If all conservative therapy do not help then surgical decompression may help.
Another root local injection can be tried.
Neurotropic medication like pregabalin may help you can take them consulting your doctor.
Physiotherapy plays important role in treatment.
