In your opinionwould the left ear need surgery? Mastoid surgery?
Brief Answer:
First nasal surgery followed by ear surgery
Detailed Answer:
Hello again,
Thanks for considering my answer and for your follow up.
In my opinion, your source of infection is from your sinuses. I would prefer nasal surgery(polypectomy + middle meatal antrostomy + turbinectomy) as early as possible and then go for ear surgery (Mastoidectomy + tympanoplasty) after a duration of 8-12 weeks.
Hope, I have answered your query.
Take care.

Did you see any indication of a cholesteoma?
Brief Answer:
Requires surgery.

In your zip file, some cuts are missing ( in depth) which shows cholesteatoma sac. Anyway, chronic case ( more than 6 months, which is not responsing to oral medications) with or with out cholesteatoma requires surgery.
Hope, I have answerd your query.
