Sir dere hyper dense extra axial collections in temporal bone ..wht significance of dese...patient is now respond on pinching verbally he is trying to open eyes by couldn't open for XXXXXXX 10 seconds..I m also dentist pls explain me thoroughly why he is unconscious yet..

In many patients, it can take time to regain consciousness due to swelling of the brain after injury. Patient slowly improves and opening eyes for 10 seconds and responding to pain is a good sign.
Another cause of unconsciousness can be diffuse axonal injury which is not visualised on CT scan. It happens due to the effect of rotational forces during trauma which causes micro hemorrhages and MRI scan can show if it is there.
There is extra axial collection which is measuring 8 mm and this causing some pressure on the brain but I think the doctors are watching and there is no immediate need for surgery. There is something called mid line shift which is to be recorded to know if there is any increased pressure inside the head and the doctors are aware of it.
In head injury, first improvement is monitored for 24 to 48 hours and then for a week. If there is no worsening and patient is showing positive response to treatment then it is good sign.
At the same time also please discuss with doctors regarding general care of the patient like nutrition, bowel and bladder, urine output and avoiding complications due to immobility like deep venous thrombosis and bed sores.

Thank u so much sir...he is regaining consciousness nw..he opens eyes for 20 second and on verbal response..now problem is cough and air blocked in lungs.pneumothorax.doctors did chest tube operation one day after x Ray's..I hope dese symptoms wl subside..doctors says dere is need of tracheostomy due to difficulty of pipes through nose and mouth..is dis fine now for tracheostomy is it nt late??? Is it complicated procedure ya lyf saving???
