What Does The MRI Report Indicate?

Question: Hello!
This is the impression after a second MRI on my left knee 3 years after ACL reconstruction: The ACL graft is felt to be intact however there is extensive myxoid change with relative enlargement of both the femoral and tibial tunnels (2 x 1.5cm in size). A septated fluid collection is seen within the tibial tunnel as described. Signal heterogeneity is seen within Hoffa's fat pad with some synovial thickening possibly resulting in some impingement here.
Do I have arthritis in the knee and that's why it fills up with fluids when I twist it?

I'm Dr Sudhir Kumar

I have gone through your query and understand your concern. According to this MRI your ACL graft is intact ( not broken). There is mucous degeneration ( fluid like collection) at both ends of ACL graft (at femoral and tibial end). Size of these is 2 *1.5 cms in size. There is also a separate fluid collection within tibial tunnel.
Fat pad below quadriceps tendon ( Hoffa, fat pad) shows difference in signals( heterogenecity) . There are also thickening of synovium(lining of joint)causing some impingement(obstruction in movements). According to this MRI there is no mention of arthritis but there is synovitis( inflammation of lining of joint) which is probably causing fluid accumulation and filling with fluid while you twist it.
You can discuss with your doctor about it.

Thank you Dr XXXXXXX for your quick response. This fluid build up from the inflamation happens everytime I do an activity that requires sideways and twisting movements (court sports, golf swing, trail running) however, I'm ok when I run long distances in a straight line. Aside from the obvious solution of not doing these activities, are you aware of any possible medical procedures not involving long term prescription (or OTC) medication? Thank you.

As there is synovial thickening so I think you are likely to be benefited by arthroscopic synovectomy and debridement by removing excess of synovitis, any loose meniscus or other loose tissue so that less fluid is formed.
