Possible degenative column disease.
Detailed Answer:
Hi, I had gone through your question and understand your concerns. From the MRI report seems there's no significant compression effect on your friend's neural structures and there's no need for surgery. There is need to better diagnose the cause of symptoms and further testing should be performed. There is need for plain radiographs (cervical and lumbosacral column) in neutral, extension and flexion position to check for/ rule out bone involvement and instability of vertebral column as well as blood work lab for any rheumatic diseases ( ESR, Fibrinogen, Rheumatoid factor etc).A possible cause may be degenative column disease. Test results should be correlated with clinical findings. After correct diagnosis treatment modalities should be clear. Your friend should use NSAID drugs ( Diclofenac if no alergic and no gastric disease), warm procedures on vertebral column. I suggest you to consult your primary care Doctor and discuss above mentioned suggestions. Hope this answers your question. If you have additional questions or follow up questions then please do not hesitate in writing to us. I will be happy to answer your questions.

EMG and Pyridoxine level should be assessed.
Detailed Answer:
Hi and thanks for asking again. Since there is a low Vitamin D level, there is need to assess Vitamin B6 ( Pyridoxine) level, and Vitamin B12, because it's low or very high levels may cause symptoms similar to those prescribed. And she should start Vitamin D supplements. Another test to assess nerve functionality is an EMG. MRI report doesn't justify that much pain. Physical therapy has a little benefit if it's done during pain period and there's no benefit if there's a toxic nerve damage. If your friend feels better walking, then that's the best physical therapy for her. Hope this helps. If you have further questions feel free to ask.
