What Does The Absence Of Periods When Suffering From PCOS Indicate?

Question: I have a long history of acute PCOS ever since I attained puberty and I am well aware of the pros and cons. I was taking Diane 35 since 2003, which used to bring in periods. however I wanted a long term cure from the problem. So started consultation with a local Ayurvedic doctor (senior and experienced physician) for the last six months. It worked well for the first cycle but never got my periods again in last 5 months. Though PMS is very strong, lower abdominal cramps, white discharge, mood swings every month and the bleeding seems probable. 2 days ago I got a blood test done for LH, FSH and prolactin. Additionally a pelvic scan and trans vaginal scan. The latter is attached. I think reveals and additional single large cyst alongwith PCOS. Please indicate the possible causes and remedies for this. Also want to know if this is related to PCOS

Sorry for the long delay in my response.
I have seen your ultrasound scan report and there is no hormonal test attached with this report.
In your ultrasound an exophytic hemorrhagic ovarian cyst is there along with PCOS but it is less than 5 cm in diameter.
It can be due to infection, inflammation, PCOS and sometimes malignant.
Usually ovarian cyst which are symptom less and less than 5 cm in diameter can be left untreated and they are resolved in 1-2 cycles but if persist after it than needs evaluation.
Exophytic growth needs CA- 125 ovarian markers to see its levels and blood test to rule out infection and inflammation.
In your case Are you having any symptoms like pain, discharge, bleeding etc?
Please let me know and if possible upload
your hormonal profile for better diagnosis and management.

I am having a slight distended pain in my lower abdomen - similar to what we get while expecting periods. My hormone values are: FSH - 4.77, LH - 8.25, Serum prolactin - 12.01. All tests including the scan have been done on May 2. Please tell me if I should go for CA-125 test now itself. Is it a blood test or a scan or both? The radiologist told me it's a fluid filled cyst similar to the ones in PCOS but is bigger in size and is on one ovary only.

I have seen your report and it is showing one exophytic hemorrhagic cyst( that is filled with blood and less than 5 cm in diameter).
As cyst is exophytic in nature, I would suggest you to go for CA- 125 marker to see for malignancy( because in exophytic growth chances are more).
It is a blood test only and not a scan.

These cyst can be due to hormonal imbalance, PCOS, infection and inflammation and sometimes due to malignancy.
Maybe auyurvedic medicine can cause cyst formation, I am not very sure about it.

I will get my CA 125 report tomorrow, will send you then. I also want to understand a bit more on how do my LH and FSH levels indicate PCOS if they are within normal range. What should it have been if the ovaries were fine?

The PCOS is complicated endocrine disorder and in this hormonal level of LH, FSH, androgen, DHEA , ultrasound scan and symptoms are considered before making the diagnosis.
In your case your LH and FSH levels fall within normal range but ratio of LH/ FSH is 2:1 means level of lh is double than level of fsh, it is usually a sign of PCOS and patient ovulation suffer because of this.
