You have a full thickness tear of supraspinatus tendon with retraction. There is also a large amount of fluid in the joint suggestive of inflammation. This cannot heal on conservative treatment. Surgical arthroscopic repair of the torn tendons is the only way.
An arthroscopic surgery can allow proper healing in 4-6 weeks. Regular physiotherapy after that will help in regaining the complete range of movement after that. Do give complete rest to your arm till then. You can use an arm sling for that.

Postponing the surgery will only add to the pain and disability. It will not help in healing. Do get the surgery done as soon as possible. Earlier the repair, better will be the healing.
