What Does The Following Follicular Scan Report Suggest?

Question: Hi Doctor,

I have taken the opinion earlier from Dr. XXXXXXX XXXXXXX please find below the interaction link:

I did a follicular scan today. Today is the 9th day. Please find attached the report.

I was taking medicine regulary. Medicines are Glycomet SR- 500, MCBM, Fertipro. Also i had Duphaston before periods then from second day to six day of period i has CCQ- 50.

Please provide your suggestion about the follicle growth and what should be the next course of action.

Hi XXXXXXX I have gone through your question and understand your concerns. Your day 9 follicular study shows polycystic ovaries and follicles of size less than 9 mm in each ovary. Endometrial thickness is of 6 mm on day 9. Both the parameters are appropriate for the day of the cycle. The medicines you are taking are appropriate. I would further advise you to continuethe follicular monitoring every alternate day till the follicle ruptures or till day 16-18, if no rupture is there.
Moreover, I would suggest you to take duphastone from Day 15 to day 26 of this cycle, as it is a progesterone which supports the pregnancy by making uterine environment more favourable for conception.

Thanks for your reply.

I will continue the follicular scan as you suggested and will start medicine as well. i was confused whether the growth of the follicle is appropriate or not since the scan doctor told that size is small.

I want to know is there any chances of pregnancy this month. Also, please suggest if any changes are required in diet or lifestyle.

Hi XXXXXXX thanks for responding. The follicular growth and endometrium is appropriate for the day of the menstrual cycle. The chances of conception in an induced cycle is maximum30-35%, and that too when the follicle ruptures and endometrium thickness is appropriate.
I would suggest you not to worry as stress also lowers the chances of conception.

Below is the result:
Day: 11, Right ovary follicle: 11x11 mm, 10X10 mm and Left Ovary is small. Endometrium is 6mm

The follicle in right ovary has minimally increased in size from Day 11 to day 14, from 11 x 11mm to 12 x 11mm. There are no signs of follicular rupture. The follicular size is less for day 14, however some women have late ovulation, and hence the size and rupture needs to be monitored till day 18-20 of the cycle.Endometrial thickness has increased.
Do follow the treatment as advised.
Do continue follicular monitoring till day 18-20 or till the follicle ruptures.

Hi XXXXXXX thanks for responding. The follicular monitoring shows increase in the follicle size of right ovary, reaching the size of a dominant follicle. Moreover, the endometrial lining has increased in thickness and has become triple lined, which is a good environment for conception.
I would suggest you to continue the same treatment and continue follicular monitoring till rupture of follicle.

Fertigyn injection is hCG hormone which is given for assisting follicular rupture and hence ovulation. The follicle has reached an optimal size and you can be given hCG to assist follicular rupture.
After hCG injection, the rupture usually occurs within 32-40 hours, and hence timed intercourse should be performed in these days.

Follicular monitoring should be done to assess follicle rupture after hCG injection.

i have not taken fertigyn 5000iu injection yesterday. Will it rapture naturally or i need to take the injection mandatory?

Hi XXXXXXX the follicle size in the right ovary has reached the optimal size for follicular rupture. However, whether it will rupture on its own or not cannot be predicted beforehand. Fertigyn injection which is hCG hormone assists in follicular rupture, and you can now take it, as it will facilitate ovulation and will increase the chances of conception.
If you are not willing for taking the injection, then you can follow up with follicle monitoring after48 hours to assess whether ovulation has occurred or not.
I would further suggest you to not take stress and be positive.

Hi XXXXXXX I have gone through your question and understand your concerns. Burning sensation in urine and itching in the vaginal region are suggestive of urinary tract infection, however vaginal infection can be associated.
I would suggest you to take plenty of fluids, continue syrup citralka two spoons twice a day in a glass of water for seven days , and get urine routine and microscopic, urine culture and sensitivity test done.
For suspected vaginal infection, you should get a vaginal swab examination done, and while waiting for results, you can keep candid vaginal pessary at night for 3 days.

Hi XXXXXXX spotting can be due to implantation bleeding, which occurs approximately4-5 days after conception, or can be due to hormonal disturbances due to duphastone, or due to vaginal infection.
I would suggest you to do not worry if there is onlyllittle spotting, however do consult your doctor if spotting is continuous or there is bleeding.
I hope that you might not have stopped taking duphastone tablets??
