Hi, thanks for posting the concern in HCM.
In stage v CKD hypertension and fluctuations of blood pressure levels are very common. The tachycardia obviously needs some assessment. But, a heart rate of 120 is not an absolute contraindication. Furthermore, labetalol is a novel antihypertensive which is free of the side effect of reflex tachycardia. So, if used for hypertension it won't increase the heart rate. But, with 200 mg substantial fall in blood pressure will be there and that needs to judiciously considered because blood pressure is not so high. You should consult your treating nephrologist for an opinion regarding the same. Assessment should also be done to find the cause of tachycardia. An Ecg should be done. If the rhythm is sinus and regular, anemia, dehydration, infection or fever need to be excluded. Therefore, you can pursue with the PD, but you have to consult the treating doctor ASAP.
