Brief Answer:
I would explain as follows:

Detailed Answer:
Hello XXXX!

Welcome on HCM!

I reviewed your son's ECG data and would explain that:

- Heart rate is normal,
- PR interval within normal range,
- QRS duration normal,
- QT/QTc within normal range,
- P-R-T axes normal

So, based on the provided information nothing abnormal could be conclude.

You should relax!

But, you should know that ECG it would be better to directly review the ECG recording for giving a more concrete professional opinion.

Could you upload that ECG recording here for a direct review?

Also, ECG is considered a helpful tool in diagnostic decision making; the most important clue to arrive to any possible disorder is the clinical symptomatology and a thorough medical examination.

Could you explain the reason of this ECG testing!

Does your son show any complaints! Or just a routine check up!

What about other possible cardiac tests! Like cardiac ultrasound (ECHO)?

Hope to have been helpful to you!

In case of any further uncertainties, feel free to ask me again!

Kind regards,

Dr. Iliri

Thank you Dr. llir for your swift response , I already attached the ECG of my son and I will also attach his echo , I made these tests because he felt chest pain while he was playing a soccer match so I went to the pediatrician and he recommended having an ECG and echo to determine the cause of chest pain it was a single incident that did not happen again up till now but I still do not know the reason why it happened

Brief Answer:
It is a normal ECG.

Detailed Answer:
Hello again,

I reviewed the uploaded ECG recordings and would explain that it is normal.

There is nothing to worry about.

There is no rhythm disorder; no atrio-ventricular conductance or repolarization abnormalities.

Respiratory RR variations are prominent and this is quite a normal finding in children and young individuals (phasic respiratory sinus arrhythmia).

So, relax!.

I would like to review his cardiac ECHO when available.

If the chest pain occurs again during physical exertion, then further investigation should be done with further imagine tests (cardiac stress test/ECHO; MRI, etc.).

I remain at your disposal case you need any clarification.

Greetings!

Dr. Iliri

Brief Answer:
Normal ECHO and ECG.

Detailed Answer:
Dear XXXX,

I passed carefully through your son’s ECHO report and would conclude it is quite a normal result.

His heart has normal structure and function.

There are no identified myocardial, valves, cardiac chambers, pericardial or great vessels disorders.

Also cardiac rhythm is normal sinus rhythm with the evidence of respiratory sinus arrhythmia, which is (as I explained you above) a normal finding in healthy children.

I would advice an ambulatory 24 hours ECG monitoring just to be sure that no any arrhythmic event occurs (which is not detected during the time of ECG recordings).

Only if that complaint occurs (chest pain) again, should be considered the above mentioned imagine tests (cardiac stress ECHO, etc.).

Hope you will find my answer helpful!

Best wishes,

Dr. Iliri

Dear Dr. lliri

Brief Answer:
You are welcome!

Detailed Answer:

It doesn't seem that Holter monitoring will reveal any unexpected arrhythmic events.

Nevertheless, it is a wise strategy to investigate the day-night heart rhythm trends for confirming whether normal respiratory sinus arrhythmia is the only responsible cause of R-R interval variability.

Meanwhile, waiting until Holter monitoring reports will be available for a direct review and a second professional opinion.

Regards,

Dr. Iliri
