The MRI Report says that you have numbness level slip disc which is closing your nerve roots which can be a cause of the pain that you may be experiencing. Annular tear means that the outer part of the disc is degenerated and torn and hence the disc has slipped and pressing on the nerves in your back.
The treatment depends on your symptoms and signs. If you have any neurological disturbances as I have asked in my queries, decompression surgery in the form of Laminotomy, Laminectomy, Discectomy may be done on the lower back. If no neurological compromise, a simple Epidural steroid injection may be given.

You could be having disc prosper in the cervical spine as well such can cause the symptoms of nerve compression.
You could get an Endoscopy done if it is hard to swallow for and experience symptoms of heartburn.
It may not all be connected to disc prolapse because gastritis and oesophageal conditions can also cause swallowing problems.
