Brief Answer:
Other conditions may be contributing to ill-health and altered sensorium

Detailed Answer:
Hello,
I have gone through your question and understand your concerns.
Altered sensorium with ventilatory support in a diabetic and hypertensive patient is suggestion of infection, encephalopathy and poor condition.
The Mri is not responsible completely for above condition.
The mild atrophy is age related. Meningiomas are of concern as it is located at two places and large size may lead to midline shift or brain herniation.
Other blood reports are required to know the actual picture.
Prolonged ventilatory support leads to chest infection and delayed removal from ventilator.
Hope you found the answer helpful.

Brief Answer:
Provide more details, not due to brain lesion

Detailed Answer:
Hello,
The brain imaging is showing incidental findings of meningiomas and age related cerebral atrophy.
The cause of unconsciousness and ventilatory dependence is not related to brain pathology and most likely due to diabetes, urinary infection and other metabolic abnormalities.
Kindly upload other blood reports, urinary reports.
Any history of seizure, weakness during onset of disease.
Any chest infection, respiratory distress, Low blood pressure?
Provide more details about initial course of disease.
