What Does This X-ray Report Indicate?
My father has got Osteoarthritis on left knee . He has the problem of knee pain almost a year long but it has become severe in last couple of months. He is not able to walk without knee pad and even with this, the walking style has changed quite a lot. After consulting Orthopaedic surgeons and with the help of X-RAY, it was found that left knee impacted with severe Osteoarthritis .
My father has gone through the treatments such as D3 supplements , Physiotherapy , tablets such as Coerip but did not find useful.
I have attached the X-RAY . Can you please verify and tell the severity of Osteoarthritis ? What are the treatments available ?
Knee replacement Surgery is the only option ? Any other methods available ? I was reading an article in web and new papers about SBF health care , a patented treatment for Osteoarthritis , which avoids surgery. Below is a text quoted by them.
"Exposure to SPMF®Therapy sensitises and re-creates the missing physiological piezoelectric stimulus necessary for cartilage regeneration.".
What is your opinion on such treatments ? Are they technically proved procedures ?
Practically what is the industry wide used method to cure Osteoarthritis.
If knee replacement surgery is the only method , any particular pre/post precautions has to be taken ?

I am Dr Gopal Goel, Orthopedic Surgeon.
I have gone through the details and x Rays. The decision to decide about knee replacement is not based on x rays or pain alone but overall clinical picture.
X Rays Show Early Osteoarthritis in your father's case , where ,in my opinion he should undergo knee replacement, since he has pain and difficulty in walking.
The treatment options are 1. medical - medicaines + exercises
2. Intra articular injections like viscosupplementation or PRP/ Stem cells which may give relief in pain and improve walking.
3. joint replacement- at his age and with pain , I would recommend Joint replacement.It is now the standard practice if indicated.
SPMF®Therapy like treatment come and go and may be beneficial for short term , but they do not help much in long term or late Osteoarthrosis cases. There is no scientific authenticity for such treatments.

As you have mentioned that it appears to be "early Osteoarthritis" , would you recommend medicines will be helpful at this stage OR is it that it has already reached to the situation of a surgery ? I know physical assessment would be needed but just would like to know from X-RAY what is the situation.
The reason to insist is that I have received mixed opinions from different doctors as part of consultation. Some doctors has said that situation is not that bad where others said that situation is severe. So confused. Well, regarding the symptoms, the pain of not continuous /not severe but yes the walking style has impacted.
What would be the type of medicines you suggest ? What would be the time frame we can wait to see the results of medicines before we opt surgery ? Also do you feel "total knee replacement" would be needed or "partial knee replacement" is enough ?

Hi I am Dr Gopal Goel Orthopedic Surgeon.
1.Early Osteoarthrosis means - that X Ray changes are not pronounced. Clinically patient may have more pain than x ray picture appears. If pain is severe and is impacting his activities of daily living then I would recommend Knee replacement.
2. Medicines I would suggest is mild pain killers , as and when required , Collagen powder to be taken once daily and quadriceps exercises to be done twice daily. Pain killers can be taken daily for 2-3 weeks in the early period but never for long to prevent side effects like damage to kidney or stomach wall.
3.In my opinion we can give an overall trial of 6 months and no improvement is noted then Knee replacement can be undertaken.
4. I would recommend Total knee replacement only.
