What Does This Abdominal Ultrasound Report Indicate?

Question: my abdomen ultrasound reveals:- pre void urine measures approx.65cc.
post void residual urine measures approx.54 cc (significant)
prostate: is enlarged in size and measures 59cc in volume. Median lobe bulge is seen into bladder base. No free fluid is seen in the peritoneal cavity. pl advise what is my medical problem?

Thanks for your query,based on the facts that you have posted it appears that you have benign enlargement of Prostate involving specifically Median Lobe with significant residual urine of 54 cc .

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Do you have irritative Bladder symptoms like Urgency for urination .increased frequency for urination ,poor stream etc?
2) Are you diabetic ?
3) Do you have nocturnal frequency of urination (Nocturia)?

Enlarge prostate is a common problem of people above the age 55 years .

Prostate consists of three lobes two lateral lobes and one median lobe through which urethral passages passes .As the median lobes gets enlarged it acts like a valve and projects into neck of the bladder. When one attempts to pass urine this projection prevents complete emptying of bladder resulting in to significant amount of urine
( Residual Urine)to remain in bladder which otherwise should get emptied completely.This residual urine becomes the source of infection and increased frequency of urination.

The enlarged prostate involving only the lateral lobes can be managed by medicine for couple of months however median lobe enlarged prostate has to be resected to make the passage free from obstruction .This is achieved by doing endoscopic resection of prostate (TURP),

TURP is gold standard method of resecting prostate gland TURP practiced all over the world and improves quality of life a patient.

I am not diabetic. My IMMUNOASSAT INVESTIGATION IS AS FOLLOW:-
PSA - TOTAL 5.35. According to another ultrasound report the volume of prostate is 84.8gms. impression: Gd IV prostatic hypertrophy with enlarged median lobe. Is it not possible to cure it through medicine? Please suggest your answer with pros and cons.

Medical line of treatment will not resolve the issue permanently.

Thanks for feed back you have not mentioned about the urinary symptoms that have .

The size of the prostate is too big (85 gms).

Normally we suggest medical line of treatment when prostate is moderately enlarged and the residual urine is 30 ml .Medical line of treatments does not stop the progress of the disease and gives improvement in symptoms to certain extent as long as patient is taking medicines ,where as endoscopic resection cures the problem for forever.

Dear doctor,
Yes urgency for urination and increased frequency for urination, and poor stream etc. noticed.
Yes I have nocturnal frequency of urination.
Dr. whether endoscopic resection will be painful or not and any side effect after endoscopic resection of prostate (TURP). Kindly advise I have to visit RG Urologist hospital Greater Kailash New XXXXXXX for treatment. Also suggest please latest method of treatment available for this.

You definitely have irritative and voiding bladder dysfunction due to enlarged prostate ..

TURP is a gold standard procedure to resect the prostate .It take 1 hour for surgery and patient is usually discharged on 3rd post operative day.
