What Does This Blood Report Indicate?
Question: feet, ankles and lower legs are swollen about 4 times normal size doctor perscribed 80 mg lasix for three days and 40 mews of potassium as blood tests showed hypokelemia this was on dec. 22, o dec 25th i had an afib attack and had to be elctro cardio verted swelling of feet are still bad and getting painful to walk primary doctor on holidday till XXXXXXX 2 and cardiologist not doing hospital follow up till january 12th, on dec.21 blood tests showed wbc 11.7

This is Dr Sameer, cardiologist.
I can understand your concern & feels like you are in CHF.
Continue taking lasix 40mg twice daily(don't take it 80mg once- if you are taking this way). Keep a proper chart of your total intake & output. Strict total fluid intake to 1ltr/ day. Avoid salt(as it increase water absorption). Keep potassium between 4-4.5. If it will be low AF will return.
Please send a detailed report of echo & KFT. We will add some more medication to your treatment which will strengthen the heart.
Also add a antibiotic like Ceftum 500mg twice daily to decrease the respiratory infection you must be having.

DON'T HAVE ACESS TO ECHO OR KFT. ALSO I DON'T EAT ANY SALT AS I HAVE NEVER LIKED DON'T EVEN USE IT WHEN COOKING, I WAS TOLD TO DRINK LOTS OF FLUIDS WITH LEVAQUIN AND PREDNISONE WHICH I HAVE ONE MORE DAY OF LEVAQUIN AND 4 MORE DAYS OF 1 X 20MG PREDNISONE THEN 1/2 OF 20 MG FOR 5 DAYS I CAN'T SEE MY PRIMARY TILL JANUARY 2, AND MY CARDIOLOGIST IS ON HOLIDAY TILL JANUARY 12TH SO I CAN'T GET THE REPORTS YOU NEED THANK YOU FOR YOUR HELP, I'M NOT SURE WHAT TO DO AT THIS POINT HOWEVER I WILL LIMIT MY INTAKE OF FLUID,

Hi,
Welcome back. Considering your condition, i expect that your heart function is less than normal. So increased fluid intake will cause fluid to accumulate in body(mainly dependent part like foot) as the hearts ability to pump it is low. So we recommend to keep a negative fluid balance (output more than intake) especially in case such as yours, where you have a lot of excess fluid.
So restrict fluid & keep taking lasix which will remove this fluid through urine. If urine output is low with it you can change it to Tab DYTOR(torsemide) 10mg twice daily which is twice potent than lasix.
If your kidney function test was normal than add Tab Aldactone 50mg once daily at 2pm, Tab Ramipril 2.5mg once daily at 9am. These 2 medicine help heart to get strong & is strongly recommended in patients such as you. One other medicine should also be part of treatment for you - beta blocker. We can add it gradually.
But keep monitoring your kidney functions & bp.

I WILL DEFINETLY TRY WHAT YOU ARE RECOMMENDING AND WILL DISCUSS WITH MY PRIMARY ON NEXT MONDAY WHEN H COME BACK FROM HOLIDAY, THEY HAVE TRIED ME ON BETA BLOCKERS HOWREVER MY BLOOD PRESSURE IS NORMALLY LOW EVEN NOW IT IS 116/78 PR 83 HOWEVER WHEN NOT RETAINNG FLUID IT IS ABOUT 100/68 SO THE BETA BLOCKERS DROPPED IT TO LOW AND MY NORMAL HEART RATE IS 64 EXCEPT WHEN IN AFIB
