Brief Answer:
Most probably brain small vessels damage from hypertension.
Detailed Answer:
Hello and thanks for using HCM.
I have read your query and understand your concerns.
Those CT scan findings are consistent with brain small vessels disease, mostly related to high blood pressure, Diabetes.
These conditions, if present, need to be treated correctly in order to stop further brain damage.
There is no evidence of your past lacunar stroke, but this depends on CT technique too.
If you experienced stroke-like symptoms, or any neurologic symptoms, brain MRI is a more accurate imaging study to assess early stroke and small strokes too.
Hope this helps. I remain at your disposal for further questions and clarifications.
Take care.
