What Does This Thyroid Ultrasound Indicate?
Right : there is a cluster of 4 hypoechoic solid module within the superior right thyroid with largest measuring 0.7x 0.9 x 0.5 cm in the posterior superior thyroid . The largest module appears exphytic and may be outside of the thyroid . There is a heterogenously echoic nodule with multiple tiny echogenic foci that may be due to colloid or may represent microcalifications . And increased vascularity within the right mid to inferior thyroid .
Left : there is a 1.2 x 1.4 x 0.8 cm hyperechoic nodule without internal hyperemia within the superior left thyroid . There is a 1.6 x 1.9 x 1.3 cm isoechoic solid nodule within inferior thyroid . There is a cluster of 4 subcentimeter hypoechoic solid nodules within the posterior mid thyroid .
1. Heterogenous solid nodule with possible microcalifications within the right mid to inferior thyroid measuring 2.6 cm maximally .
2. Isoechoic solid nodule within the left inferior measuring 1.9 cm maximally .
3. Hyperechoic nodule with in the left superior thyroid measuring 1.4 cm maximally .
4. FNA of all three nodules is recommended .
5. In addition there is a 0.9 cm hypoechoic nodule that may lie posterior to the right thyroid . This structure may represent a small exophytic thyroid nodule or an enlarged parathyroid gland . Parathyroid hormone measurement is recommended .
In addition follow up sonography in 6 - 12 months is recommended .
Echogenicity Echotexture and Vascularity : Normal with homogenous Echotexture . There is diffuse hypervascularity bilaterally .
I suspect that this is saying there is malignant nodules and I am extremely upset with this report and no way to interpret it .
FNAC can assess true nature of nodules in majority of cases.
Detailed Answer:
Hello
Your findings suggests multiple nodules in both lobes of thyroid. Few nodules contain echogenic structure within which may represent micro calcification or colloid. We need clinical details, thyroid profile test, thyroid radionuclide scan and importantly FNAC to understand the nature of nodules and rule out malignancy. Radionuclide scan has been done already in your case. It is important to know its result. It can comment on hot or cold nodule which helps in diagnosis.
With just the scan details, focal thyroiditis and tumours are couple of possibilities that should be considered. But again, it is too early to comment and worry about malignancy. I will suggest you go ahead with the doctor appointment. Discuss about radionuclide scan result and plan FNAC.
Besides thyroid nodule, there is also doubt about enlarged parathyroid gland. This also should be corroborated by blood and clinical findings.
So,you should for FNAC,it will guide further treatment.
Hope I have answered your query.
Further queries are most welcome.
