What Does This Ultrasound Report Regarding Hepatitis B Infection Indicate?

Question: Hi
I am worried about my father who has hepatitis b for more than 10 years. His liver function is almost normal (one reading was slightly high). Ultrasound showed :"enlarged liver, the echotexture is diffusely coarse but with no focal solid lesions." I am very scared about this and worried that something seriously wrong is going on. We are waiting to see a liver specialist. I am really worried and can't wait , please help by clarifying these results. XXXXXXX

Hepatitis B infection over a period of time can cause liver cirrhosis and liver cancer. Hence periodic follow up with the doctor is very important.

The liver function test is normal and there is no focal lesion now. This means the liver is not damaged by the virus and there is no liver cancer.

He needs to avoid alcohol.

Further work up for hepatitis B infection has to be done.
e antigen and HBV DNA tests and a fibroscan are to be done to decide further treatment.

Hi,
Liver finding in ultrasound scan is reported as echotexture.

As per his report, his liver is enlarged and the echotrexture is altered. It could be due to fatty liver or fibrosis/early cirrhosis.
The fibroscan I have advised in my previous post will help to know whether there is fibrosis.

Thanks alot for your reply. As I said earlier I am extremely worried so please in general how do you assess his liver. Is enlarged liver and fibrosis something to worry about. I forgot to mention that he didn't recieve any treatment for heb b before as doc thought it is not needed.

Hi,
If it is just fatty liver and the liver panel is normal, there is no need to worry.
But if it is fatty liver with raised liver enzymes or fibrosis, then it is a thing to worry about as over a period of time it can raise the pressure in the blood vessels of liver causing complications like bleeding.

In my first post I have mentioned that more tests are to be done to plan treatment for Hep B. If all those tests have been done already and if test report indicates there is no need for treatment, then periodic follow up should be done.
An ultrasound scan and serum alpha feto protein test also should be done every 6 months.
