Question: Hello, I have had frequent UTI's over the last couple of years, and kidney infections. I do not have stones. I have been through the medical tilt-a-whirl repeatedly. I currently have a 100-101 degree fever for over 10 days now. I went to the ER. My urine has a lot of red blood cells, white blood cells, leukocytes, pus, etc. I am diagnosed with a UTI. The culture comes back today as negative. No infection. I also still have constant pain on the right side of my abdomen, and on my back. Could this be a kidney infection?

1) You definitely have urinary tract infection as per report.

2) Sometimes it is not seen in culture and sensitivity test as it is sterile pyuria mening sterile pus which cannot be cultured.

3) It could also be due to laboratory error or collection error.

4) Please repeat the culture and sensitivity test from another standard laboratory with early morning urine specimen.

5) This might come positive.

6) Since you are suffering from high fever i suggest you give urine for culture and sensitivity in another laboratory and you get prescription for paracetamol and broad spectrum antibiotic from your doctor as you have no time to wait for another culture report.

7) Request your doctor to start empirical antibiotics as per clinical experience till another sensitivity report comes.

Thank you for your response. I do have another question though. Why do I continue to get UTI's? I have had over 40 in 3 years. I also suffer from endometriosis. I have had this surgically remedied twice. The last time, the surgeon got too close to the ureter and that's how I ended up with a uretal stent for 5 weeks. Everyone is so quick to blame it on the endo, but my gynecologist has never seen a patient like me due to endo.I have had many Ct's, MRI's, etc. I am scheduling an IVP this week. Any other reasons you can think of why I would have this kind of problem?

Endometriosis is in no way concerned with UTI. Its different altogether.

This repeated UTI is probably due to the stent and ureteric injury episode. This might have formed some form of infection focus in ureter or there could be kinking or disfigurement of ureter leading to multiple UTI.

Thank you. Last question. I have been on antibiotics for almost a week now. I am no better. Still have a constant 99°- 100° fever. Lots of pain. This feels different than my normal uti. Could this be an upper tract infection now? More like a kidney infection?

It is difficult to say if it is upper tract infection or lower tract infection.

Either way it should show up on urine culture. But the status is as discussed above.

Get another sample and lets see the culture report.
