What Does Weight Lose When On TB Treatment Indicate?

Question: My relative (84 years) diagnosed with Type A- TB (TB -PCR test is negative) & he is receiving treatment with AKT 4 kit- Doctors have told us to get the treatment for 6 months. During first 2 weeks of treatment he lost 3 kgs. My question is there any possibility that the medicines can be reduced & what to do for weight gain.

I can understand your relative 's situation and problem.
There is no term like type 1 TB.
So I need to first know the symptoms of him.
Is he suffering from Pulmonary tuberculosis or extrapulmonary tuberculosis?.
The treatment duration for tuberculosis is at least 6-8 months.
And we can reduce drugs only after 2-3 months of treatment with clinical signs of recovery.
The first clinical sign of improvement is weight gain and subjective wellbeing. Since you are telling that he is not improving, rather lost 3kgs in 2 weeks of anti tb treatment, we need to revise the diagnosis.
So please tell me
1. Which symptoms he is having?
2. Since how many days he is having these symptoms?

He had only chest pain for that we admitted him, There was no problem in ECG & In Angiogram reports only 5-10% of blockages was there some drugs they prescribed. He took for 3-4 days. After getting discharged we admitted him again after 6-7 days since he was suffering from loose stools & vomiting. The doctor said it was some hospital infection which might have acquired during his first admission. later on after chest X ray & CT scan it was diagnosed as Pneumonia & the doctor said it was pleural effusion . After 2-3 days of antibiotics about 400 ml of water removed from right lungs & sent to lab. It was negative & TB PCR test was also negative. He has been tested for HIV & it was also negative. His sputum also sent for TB bacteria test & it was negative. On the basis of blood test the doctor told us that he has TB (The doctor told one of the blood count has to be less than 50% & the reports say it is 80%) Then he started treatment for TB. The doctor said that this particular TB is not going to transmit to others since his sputum test negative I need to know on the basis of which test the doctor has come to conclusion that he has TB ( The doctor said on the basis of blood reports). Whether this is transmitting disease. Following drugs prescribed to him.

He has taken forcan (Daily morning one tablet for 2 weeks & doctor told him to stop)

Morning 1 tab for gastritis before food
Benedona - Half tablet in afternoon.
Night - AKT4- daily 1 kit
Since last 6 months there was frequent fever & there was no cough. One thing I know he lost some weight during last 1-2 years about 7-8 kgs. He said he is purposefully avoided food to bring down his weight. I think his weight was about 58 kgs about one year back. At the time of 2nd admission he was around 52 kgs. On discharge 49.60 kg. After two weeks of treatment 47 kgs.

The blood report you are talking about is mostly lymphocytes.
It is one of the white blood cells (WBCs). Normally it is less than 50%. But in chronic infections like tuberculosis or fungal infection, lymphocytes are high.
If it is tubercular pleural effusion then TB PCR must be positive. TB PCR is highly sensitive and specific for the diagnosis of TB pleural effusion. So if it is negative, we need to think of other causes, other than tuberculosis.
So please tell me
1. Color of the aspirated fluid.
2. Which other tests of fluid have done?
3. Has doctor tested for malignancy?
