What Doest This Abdominal Ultrasound Report Indicate?
Question: My friend who is 46yrs male, approx. 58kgs with 5.7" ht has been heavy alcohol consumer and not been eating food for a week. Was admitted for treatment and his Ultra Sonograpghy of Upper Abdomen reports are as follows:
Liver - Mildly enlarged in size. Diffuse fatty infiltration noted.No focal SOL seen. Intrahepatic biliary channels are not dialated. PV measures 9.1m.m. in calibre.
C.B,D. - Normal. No SOL. measures 9.5m.m.
Pancreas - Normal in size. Echotexture is slightly heterogenous. No focal SOL. Pancreatic duct is not dilated.
Spleen - Normal is sizs and position. No SOL
Both Kidneys - Normal in shape, size and position. Corticomedullary differentiation is seen normal bilaterally. Pelvicalyceal systems are seen normal. No SOL. Rt. kidney measures 104 m.m. Lt.kidney measures 111m.m
IMPRESSION: U.S.G. study of upper abdomen shows: 1. Mild hepatomegaly with fatty infiltration. 2. Mildly distended gall bladder with biliary sludge in the lumen and 3. Multiple cholesterol deposits in the gall bladder wall.
What should be the next course of action? Please advice. Warm regards,

I read carefully all your concerns and I can say as follows:
Abdominal ultrasound shows mildly distended gall bladder with biliary sludge.
It means that he needs treatment with biliary acids and antibiotics.
Otherwise If I were his treating doctor I would recommend to do as follows:
- abdominal MRI to better evaluate biliary tree
- blood work
- lipidic profile
This is why to my opinion he needs to consult with gastroenterologist and get the proper treatment with biliary acids and probably ( depend of blood work) antibiotics .

I read carefully all your concerns and I can say as follows:
Abdominal ultrasound shows mildly distended gall bladder with biliary sludge.
It means that he needs treatment with biliary acids and antibiotics.
Otherwise If I were his treating doctor I would recommend to do as follows:
- abdominal MRI to better evaluate biliary tree
- blood work
- lipidic profile
This is why to my opinion he needs to consult with gastroenterologist and get the proper treatment with biliary acids and probably ( depend of blood work) antibiotics .

For now, what I understand is the advice of removing the gallblader rules out and can be treated with medication. RIGHT!

Hi back,
It is important to do the tests I mentioned above to definitely indicate the best choice of treatment .
after MRI will be decided if he has stones or only biliary sludge.
If it is only biliary sludge than medical treatment with biliary acids is the first line of treatment for 6 months.
If there is biliary stones than surgery is the only choice of treatment.
