What Is Causing The Pain In The Uncovered Glans? What Precautions Should Be Taken?

Question: I got married 9 months back and am leading a sexually active life since marriage.My foreskin has slipped below the Glans and it doesn't fully cover the Glans. The portion of Glans which remain uncovered produces high sensitivity and pain when it touches the underwear or anything else. I have to modify my walking style as well as sleeping style so that the uncovered portion of Glans doesn't touches anything.

Can you answer these questions
1) What can give me immediate relief against this sensitive uncovered glans portion?

2) How much time normally Glans take to become completely insensitive to external conditions? or will it ever happen that Glans become completely insensitive?

3) What should be my future approach? Shall i allow my skin to get completely retracted back and Glans to become totally uncovered. or shall i make sure skin keeps covering it as much as poosible? Which is better for a healthy sexual life?

4) What are the hygiene measures i should follow for Glans and foreskin?

To answer your questions...
1) The main cause for pain in the uncovered glans is dryness. When the glans is covered by the prepuce (foreskin), the secretions on the inside of the foreskin prevent it from drying. So, the best way to get immediate relief is to apply a mild moisurising agent. You can apply a thin layer of vaseline over the uncovered area two to three times a day (being careful not to apply inside the urinary opening). Avoid using any perfumed or strong creams or mositurizers as the chemicals may irritate the skin. So, just use plain vaseline. If the pain is intolerable, then you can use a a very thin layer of a mild anaesthetic gel, like 1 or 2 % Xylocaine gel.

2) It may take a at least few weeks for he glans to become insensitive to the external friction or rubbing against clothes. It cannot become completely insesitive, since some amount of touch sensitivity is essential for both sexual and non-sexual purposes. But it can become insensitive enough to not feel discomfort or pain.

3) Regarding your future approach, it is better to just leave the foreskin it as it is and not retract it completely behind the glans. This is because in some persons, if the glans is fully exposed, the constant friction can result in darkening or thickening of the skin over the glans. Also, there is a chance that the retracted (and bunched up) foreskin may accumulate dirt and dead cells and may result in a hygiene problem also.

4) Regarding hygiene measures, it is better to wash the penis area frequently with a mild soap and water, especially after sexual intercourse. While washing, you can retract and pull back the foreskin a few times - so that the foreskin becomes more elastic and covers the glans properly. Avoid sweat or dirt accumulation in the genital area. Use thin cotton underwear to avoid sweating and for better ventillation. This will also help minimize the hypersensitivity over the exposed glans. Avoid using strong prefumed soaps or shower gels or antiseptic solutions to wash that area.
