Brief Answer:
First medial therapy, then consider surgery.

Detailed Answer:
Hi,

Thank you for your query.

1. The images of the scan done in November 2014 show a mild DNS (Deviated Nasal Septum) which is very common.

2. There is bilateral inferior nasal hypertrophy, more on the right and a small right sided concha bullosa of the middle turbinate.

3. Take a trial with a few weeks of medical therapy including an anti-histamine, an anti-leukotriene, a steroid nasal spray, hyper tonic nasal saline washes (Nasowash and Physiomer) along with regular steam inhalation.

4. The need for a septoplasty and the reduction of the concha is not convincing. Upload axial images. If the medical management fails, consider Radio-frequency or coblation nasal turbinate reduction.

5. You may consult Dr. XXXXXXX XXXXXXX at his private clinic or Dr. XXXXXXX Vishwakarma at the BJMC & Civil Hospital XXXXXXX The cost will vary between the private hospital and government institute. Between these two options your expected cost will be easily available.

I hope that I have answered your query. If you have any more questions I will be available to answer them.

Regards.
