Yes, as it is very clear with your scan as suggested by your ENT surgeon - all the factors outlined will be responsible for your breathing difficulty. Primarily, this is due to the fact that all the mentioned points reduce the airway efficiency mechanism.
The priority for your ENT doctor now would be to treat you with antimicrobial agents for your
maxillary sinusitis (thickening) after culture and sensitivity tests. After this infection subsides - couple of months later on follow up septum and turbinate defects will be looked into as much as the maxillary sinus. All these 3 mentioned will be considered for treatment even if a surgery is mandated.
COPD (assuming "mild OPD") can also be the next important reason for breathing issues - but more light will be shed in the pulmonary function tests. The type of COPD will also have to clinically determined.
