Thanks for following up. The poking sensation that you have mentioned is usually described as a throbbing sensation. It usually occurs as part of symptoms of anxiety. It can also occur in response to stress.
Many people do have similar headache as part of vascular headache like migraine. In your case, it is clearly associated with other anxiety symptoms.
As I suggested, increase in dose of escitalopram and relaxation exercises will help reduce the symptoms. In addition, option of adding propranolol can be considered which will help in reducing the symptoms of anxiety as well the throbbing headache.
