I have noted your diagnosis and medications tried.
Hemifacial spasm is caused by involvement of facial nerve, which start in the brainstem and come to supply the facial muscles. It could be due to compression of facial nerve by a blood vessel. in some cases, no causes can be found.
Though several medications are used for treating hemifacial spasm, including pacitane (trihexiphenidyl), and other medicines which you have used, the best treatment is botox (botulinum toxin injections). Botox is injected directly in the affected muscles over face.

Botox injection needs to be repeated once in 3-4 months and can be given several times. In one patient, I have given botox 14 times over a period of five years for hemifacial spasm. The effect of botox injection is temporary (3-4 months) and not permanent.
Surgery is not the best choice. You can choose medical treatment or botox injection.
