Small vessel disease is one particular type of disease affecting your blood vessels and is a major form of vascular dementia.
Although small vessel reduces life expectancy to about 50% of normal, by four years from initial diagnosis I would say this varies from person to person.I were treat hypertension(important risk factor next to ageing) adequately & this has shown to improve the quality of life and survival.

I have read that a person will usually experience steps downward with Small Vessel Brain Disease. Is there an average time period between these steps....maybe small strokes....and does death come usually from a massive stroke?

Small vessel disease peaks during seventh decade.The decline in the functional ability is due to insult to the brain cell in focal areas called as ischemic stroke as rightly put by you.The frequency of these events are not regular and hence donot have a definite time interval between successive episodes.
Death occurs secondarily to chest infections(pneumonia) due to disability/in case of being bed ridden and at times due to a massive stroke.
