I have noted your symptoms over the past seven years, the treatment received so far, and also the details of the surgery done in 2006. I have also seen the three
MRI reports attached with this query.
It is unfortunate that your pain is persisting despite surgery, medications and
physiotherapy.
I would like to point out that now as per the latest MRI report, there is a
nerve compression at L3-4 level, which is different from the L4-5 level, where the original surgery was done. So, in a practical sense, we are dealing with a new disease.
The treatment would initially consist of medications such as
gabapentin and
pregabalin, physiotherapy, etc.
Epidural injections may be given if the above does not help relieve the pain.
If both the above fail, then surgery would be needed. However, I would agree that second surgery is more challenging, and it should be done by an experienced
spine surgeon.
