At What Age The Third Dose Of Pentaxim Be Given?

my baby is 4- month old.
She got her 2nd dose of pentaxim vacination on 18 Jan,2014.
Could you please tell me when should the next dose be given?
And also tell us what is the extended limit for giving this dose, as the vaccine is not available in the market.

Ideally the gap between two doses should be 4 weeks. If the same could not be done for some reason at the earliest possible.
