Can I Increase The Dosage Of Midodrine For Low Blood Pressure?

My husband was prescribed midodrine HCL 10 mg for low blood until was have his appointment with a electrophysiologist, with will happen on 6 days. This medication was prescribed by his nephrologist.. After we informed his cardiolgist of this prescription, he said, it is supposed to be taken 3 times a day and if you listen another doctor, that means that you do not trust me and from now, I will not take care of him. Now, the next appointment with the cardiologist is in 10 days. Until then, can we increase the midodrine. It is very effective bringing his b/p from 80/50 to 100/80. Mu husband has; CHFF, Atrial fibrillation, he is on peritoneal dialysis as his ejection fraction his 15. Please your opinion. I am a RN and know that it will be only an opinion not a prescription.

Cardiologist 's  Response
Hello! Thank you for asking on HCM! About midodrine dosage, it is normally used 10 mg three times a day. Although its metabolite (desglymidodrine) is dialyzable a dose reduction is needed in chronic renal disease. I don't know your actual renal function (creatinine clearance) but you have better to talk with his. nephrologist Regarding you arterial hypotension I recommend you to discuss with the cardiologist for any possible modification of maintenance therapy (he suffers heart failure, atrial fibrillation.
Hope to have been helpful.
If you bring detailed information about his therapy I could give a more concrete opinion. Greetings from Dr. Iliri
