I carefully read your question and understand your concern.
Generally the gynecological visit in the first trimester of
pregnancy is not found to cause
miscarriage. I understand your concerns with cervical movements and the rest of examination but all that is normal parts of a gynecological examination and are not supposed to cause problems to pregnancy. The scratch is a strange sensation you mentioned as the doctor should have had gloves which should not allow her to cause scratching.
Sometimes we even do pap smears in the first tremester if the woman has not done one in the last 3 years, which means we insert e brush in the cervical ostium during this procedure and still it should not cause pregnancy loss.
Mostly in the first trimester the causes of pregnancy loss are not mechanical but genetic, the development is not going well and the pregnancy is interrupted.
I am not sure in your case if the pregnancy was confirmed: 3 weeks after the LMP the woman does not know yet she is pregnant and you are right the tests very likely will not show pregnancy. In this case it is more like the bleeding was the coming period as usual.
Generally a woman should miss a period and after that she realizes she is pregnant and tests come out positive. In the case of missing a period the woman is not 3 weeks after LMP but 4-5 weeks and the tests should show pregnancy.
So to close it:
1. the procedure is not found to be harmful in the first tremester of pregnancy
2. the causes of pregnancy loss in the fist trimester are mostly genetic
3. I am not sure in your case the pregnancy was diagnosed or you were too early.
4. If you were pregnant in a way this is a good news as this means you can conceive again
