Can Uterine Fibroid Cause Abdominal Cramps And Lower Back Pain?

At 25, I had surgery to remove 2 dermoid ovarian cysts (7 cm & 4 cm) 6 months ago. I have never been pregnant and had no previous issues until pain in lower right side began. During the laparoscopic surgery, he noticed 1 small fibroid, but did not have time to remove it and believed it too small to be a bother. Since my surgery, my periods have been very painful, almost unbearable. My OB/gyn has done 2 transvaginal ultrasounds and said ovulation was likely irritating my healing ovaries. I began Ortho Tri-Cyclen Lo to prevent ovulation. Started feeling better 3 months after surgery, then got my period and started experiencing horrific cramping, lower back pain, and muscle guarding throughout pelvic/lower abdominal region. The pain interrupts my sleep, even when I take 800 mg Motrin. It seems worse when lying down and sitting. I have seen 2 OB/gyn docs and my primary care, who wants me to go to Physical therapy and says it's a muscular problem. The OB/gyn second opinion said I should take Depo-Lupron shots to shrink any fibroids, but the shots are extremely expensive and menopausal effects at the age of 26 are undesirable to me. I just want someone to tell me for sure if my tiny fibroids are causing me this much pain constantly. Also, my primary care will not order an MRI to check my lower back to be sure, but I had a lumbar x-ray, which of course, showed nothing. He does not want to see me again until I complete the PT he suggested. My OB/gyn says on u/s he can see 2 small fibroids that should not be bothering me as they are just so very small. He thinks my condition will just improve over time, but it seems to be worsening. I am considering a 3rd GYN consult. Please advise.
